In this article, we’ll guide you through the process of accessing and utilizing this innovative tool, which eliminates the need for physical paperwork, scanning, and manual data entry.
Step 1: Verify Veterinarian User Profile
Before you can begin using the digital health examination form, ensure that your veterinarian has a user profile in your UKM system. If you’ve already created one, make sure it’s active and accessible. Your vet will need their username and password to log in.
Step 2: Access the Form
Once your veterinarian’s profile is set up and ready, log in to UKM using their credentials. You’ll sign in as your veterinarian, allowing you to perform the examinations on their behalf.
Step 3: Check Puppy Status
Before starting the examination, confirm that the puppies you’re examining are listed as either “pending” or “coming soon” in your UKM system. These statuses indicate that the puppies have not undergone the vet examination yet.
Step 4: Begin the Examination
To start the examination, click on the specific puppy you’re examining, then navigate to the “Health Examination Form.” Here, you’ll find checkboxes and fields to record various aspects of the examination.
Step 5: Record Examination Details
Fill out the form by selecting the appropriate statuses for each section of the health examination. If there are changes or additional notes, simply touch the screen to make adjustments. You can document any comments in the provided field.
Step 6: Veterinarian’s Signature
Once you’ve completed the entire examination form, your veterinarian can sign it directly on the iPad. After the signature is added, click “Submit” to finalize the examination.
Step 7: Review the Examination
You can view the completed health examination within the Pet Counselor view. The examination displays each section with the chosen statuses and any comments. The veterinarian’s signature, along with the date, is also included. This digital examination is accessible anytime, anywhere, eliminating the need for physical copies.
Step 8: Printable Document
If you ever require a hard copy of the examination, you can easily print the digital document for your records.
